2020 ENROLMENT FORM

This Enrolment form has been designed to gather information about your child to assist us in supporting your child's transition to kindy. Additional information and parental consents are also required to meet legislative requirements. We also ask that you read the Parent Information Handbook which will be provided to all families. The Handbook will provide further information about our policies, fees and procedures at Holland Park Kindergarten (HPK).
Please note the following:-
1. We will require details of persons you authorise to collect your child and also persons you nominate as emergency contacts. Please ensure these persons are aware you have listed them on this enrolment form. You will need to supply their Name, Phone Numbers and Driver's Licence Numbers, so please gather this information BEFORE you commence completing this online form.
2. Please allow approximately 20 to 30 minutes to complete. If necessary, you are able to return to the form, prior to submitting the form, if you open it on the same device and using the same browser it will retain previously entered information.
3. If any required fields* are Not Applicable to you, please write N/A in the required field.
